Start an award nomination

 (Note: Any paid member has the ability to nominate another paid member for an award.  You will have to notify if you nominate someone else for an award. They will have to indicate they will be attending the banquet in order to receive the award and mark it as completed.)

 

  • Start at the Award Applications Home Page
  • Under "Start a new award nomination" select the appropriate award
  • Search for the member for which you want to nominate
    • Enter last name
    • Continue

 

Printed name to appear on award: How the name should appear on the award.

 

Phonetic spelling of name: How the nominee's name is to be pronounced.

 

Years of CES Service: Tenure as of March 15 of the year of the award.

 

Agree to attend Annual Banquet: Nominee must attend banquet in order to receive award. This field can only be edited by nominated the member or state chair.

Text abstract, citation, plan or entry:

Enter the appropriate text (you can copy and paste from another application)  for the award.  This may range from 75 words for the AA & DSA citations to 1200 words for a Search For Excellence application.

 

Citation: The citation statement to be read at the awards banquet. Limited to 75 words.

 

Make release directly to media: Choose to have release made directly to media.

 

Media Contact information: Contact information for up to two media outlets.

 

Press release: This is what will be sent to the media and can include more than 75 words- if you selected "Yes" for "Make release directly to media".  If you leave this blank when you save the application for the first time, the text in the abstract/citation/plan/entry will be used.  Future editing of the abstract/citation/plan/entry will not change the "Press release".

 

Save - Click to Save information for future editing and to continue to summary page and to upload photo and any supplemental files required for this application.

Review your application information.

 

Upload any supplemental files. Be sure to give a description or title of the file being uploaded.

To replace the file, upload a file with the same name as the one you uploaded first.

 

Upload your portrait from a JPEG file.

    Max size = 2MB

    Recommended resolution: 480 pixels x 640 pixels

Select the image file by using the Browse (which will allow you to browse photos on your computer).  Once you have selected the photo, you will need to select the button (Send File)

 

Application is complete: When the application is complete, select "Yes".  This is only an option after all fields have been filled.  THIS MUST BE changed to YES before the state chair can approve this application.  Only the applicant can indicate that the application is completed (with the exception of the Hall of Fame award).

 

Once an application is marked as complete, you may change it to "Incomplete" and continue editing by returning to view this application.  If the appliacation has been approved at the state, regional or national level, any changes will require the application being re-approved at all levels. It is your responsibility to notify the appropriate committee chair.

 

 


 

 

State Committee Chair Actions

 

State Recognition and Awards Committee Chairs will have an extra section to the Award Applications page.

 

If you are the Recognition and Awards State Committee Chair, you will be responsible for the following:

 

  • Review award application for acuracy, make sure they are complete and meet the guidelines for the award.
  • For AA & DSAs, get approval from the State Extension Director for these nominees to receive the award for which they are nominated.
  • Transmit the winners to the Recognition and Awards Regional Vice-chair.
  • Mail (or email) confirmation of transmittal and State Extension Director approval to the Recognition and Awards Regional Vice-chair.

 

Review award application for acuracy, make sure they are complete and meet the guidelines for the award.

 

If the application is complete, the nominee's name is a link to their application summary. 

 

If the application is incomplete, the nominee's name is a link to their application and you have the ability to edit it.  This privilege is afforded you so that you may help the applicant.  Any changes should be approved by the applicant.

 

If the application does not meet the guidelines or needs to be changed, click on the "Change to Incomplete" button on the nominee's application summary page. Then, contact the nominee to explain what needs to be changed and why. The applicant will receive an email with the reason you indicate.

 

 

Get approval from the State Extension Director for these nominees to receive the award for which they are nominated.

 

  • Select the state nominees
  • Click on "Create Extension Director Approval Message"

Do either or both of the following:

  • Insert the State Extension Director's email message in the Email Address field.
  • Edit the message as appropriate.
  • Replace "INSERT STATE EXTENSION DIRECTOR'S NAME HERE" with the State Extension Director's name.
  • Click on the "Email Extension Director" button.
  • Follow up to make sure the director received the message if you don't get a response in a reasonable amount of time. (Messages sent from this form may be flagged as spam and not delivered)

OR

  • Copy the text of the message into an email message using your favorite email client.
  • Send the message to the State Extension Director.

OR

  • Copy the text below the "Email Extension Director" button and paste it into a word processing document.
  • Edit the message as appropriate.
  • Mail the letter to the State Extension Director.

 

Transmit the winners to the Recognition and Awards Regional Vice-chair.

 

  • Select the state nominees
  • Click on "Transmit Winners to Regional Vice-Chair"
  • Click on "Approve Transmittal to Regional Vice-Chair"

 

 

Mail confirmation of transmittal and State Extension Director approval to the Recognition and Awards Regional Vice-chair.

 

  • Print the Transmittal Sheet
  • Mail Transmittal Sheet along with proof of the State Extension Director's approval to the Regional Vice-chair (address is printed on the Transmittal Sheet)

 

 


 

Regional Committee Vice-Chair Actions

 

 

Regional Recognition and Awards Committee Vice-Chairs will have an extra section to the Award Applications page.

 

If you are the Recognition and Awards State Committee Chair, you will be responsible for the following:

 

  • Review award applications for acuracy, make sure they are complete and meet the guidelines for the award.
  • Verify the State Extension Director's approval has been received for each applicant
  • Transmit the winners to the Recognition and Awards National Recognition and Awards Committee Chair.

 

When the state winners have been approved, you will see the nominees listed under each state and grouped by awards.

 

Review award applications for acuracy, make sure they are complete and meet the guidelines for the award.

  • Click on each nominee's name to view their application.
  • If there are any errors or changes that must be made, click on the "Change to Incomplete" button and  follow up with the state chair.
    • Incomplete applications will need to be marked as completed by either the applicant or state committee chair.
    • After being marked as complete, it will need to be re-transmitted by the state chair.

 

Verify the State Extension Director's approval has been received for each applicant

  • Review the approval documentation sent by the state chair

 

Transmit the winners to the Recognition and Awards National Recognition and Awards Committee Chair

  • Select the state winners that are to be transmitted to the national committee chair.
  • Click on "Transmit Winners to National Chair" button.
  • Review list
  • Click on "Approve Transmittal to National Chair" button.
  • Print the Transmittal sheet and mail OR email the Transmittal page to national committee chair.
